Pre Participation Physicals
All Ventura College Student Athletes need a completed Pre-Participation Physical before they can compete. The information below will help you navigate this process this year.
- If you are a student athlete that is a first year, third year or new athlete to Ventura College we request that you try and get a physical with your MD (insurance permitting). Especially if you are a local student-athlete. All forms can be found in the next module. Once all the forms are completed the student athlete they can scan and email back to Tyler Hickok and Nathan Donnelly our Athletic Trainers.
- If you are an out of state student athlete and has insurance you are strongly encouraged to get a physical in your state . All forms are attached. Once all the forms are completed the student athlete then can scan and email back to Tyler and Nathan. thickok@vcccd.edu and nathan_donnelly1@vcccd.edu
- If you are a second year student athlete you will be allowed to get a physical from the athletic training staff but in small groups on dates TBA. IF you would like to get a physical on your OWN, the athletic training staff encourages that too. Please follow the directions that a 1st year would have to do to get a physical.
- If a first year, third year or new student athlete cannot get a physical the athletic training staff will provide a date and time for you to get one through us. The dates so far are August 4th for Football & Women's Soccer and August 25th for Cross Country, Water Polo and Volleyball. Each date is limited to 50 time slots and the student athlete will have to sign up for a time using Sign Up Genius (will email link and time {probably between 3:30 pm -7:30 pm} once Executive Team approves our plan). The student athlete will need to complete the forms before coming on their assigned date and time or they will be sent home. For M/W Basketball, Baseball and Softball we are working on an early September date. For all other sports we are working on a date later in the Fall.
- All athletes will be required to fill out a COVID pre-participation and assumption of risk form. If you have had symptoms and received medical attention you will need documentation or an MDs signature in order to clear you for competition.
- If an athlete is a minor they must have their parents fill out the minors consent form.
