Template: Student Surveys

Surveys can serve an important function in an online class. It is suggested that faculty include two types of surveys in their online courses.

First, an anonymous Learner Feedback Survey can help the instructor get valuable student feedback on their learning materials, instructional methods, course organization, and more. Ensuring that this survey is anonymous will solicit more honest responses from students. It is recommended that feedback surveys be administered at least once during the course and again at the end of the course.

Second, a Learner Self-Assessment Survey is a great way to help students determine if they are practicing good habits to be successful. Instructors are encouraged to review the results and provide resources and support to students as appropriate. Although this survey is mostly for the benefit of the student, in reviewing the results, instructors may gain awareness of instructional improvements that could be made to increase student engagement and success. It is recommended that self-assessment surveys are administered early in the course to set students up for a successful term and again toward the end of the course to help students be more successful in future courses.

Administering these surveys throughout your course shows students that you care about their success and their learning experience.

Below you will find examples and templates for both types of surveys. Be sure to edit the highlighted and bracketed text when using the template!
